Abstract                                                                                                                 

Title: Management of Postero-medial proximal Tibial plateau fracture by Postero-medial plate Osteosynthesis: An objective as well as subjective functional assessment at the tertiary level

Materials and methods:

Retrospective as well as Prospective study

INCLUSION CRITERIA:

All skeletally mature patients

EXCLUSION CRITERIA

1 Fracture in skeletally immature patient

2 Patients with advanced osteoarthritis

3 Rheumatoid arthritis or other inflammatory disorders affecting the knee.

4 Patients with pathological fractures

5 Pregnant female patients

6 Compound injuries grade II and III

Age group: 18 to 60 years

Total No. of cases: 23 (M:15, F:8)

Evaluation: clinical & Radiological- plain x-ray (AP & Lat.), CT scan (axial, sagittal, coronal)

Diagnosis: All Schatz. IV, V, VI with Postero-medial fracture fragment

Approach: Isolated Postero-medial or Combined with Antero-lateral in SUPINE

Implants: T or L buttress plate

Total follow up: 1 to 5 years (avg-3 yrs)

RESULTS

Final ROM: Avg. 122.3 deg.

Total follow up: Avg. 36.5 (18-56)months

Wound healing: Avg. 12.73 days (18-18days)

Period of union: Avg. 4.13 months (3-5 mnths

Conclusion

Fracture is vertically shearing unstable fracture pattern,needs absolute anatomical reduction through direct postero-medial approach.

4.5mm ‘T’ or ‘L’ buttress plate is the ideal implant
