All three major nerves of upper extremity have shown most unusual and sometime unreported injury patterns following a fracture management with implants and instrumentation.. Similarly, Kirschner wires have damaged and devascularised digits, sometimes irreversibly, with most bizarre methods of fixation.

While some of these complications have been reported earlier, a compilation of all these damages has not been reported. The compilation is of significance because implants and instrumentation are used by all of us but bring along with them, a lurking danger which is right there in the trajectory of the metal. Inappropriate used, healing end of the metal (Implant and instrumentation) may become a bayonet of damage piercing the soft tissue in its way.

All these are preventable situations. The paper discusses 1. Why the damage occurred?  2. How it was surgically managed? 3. How to avoid them?

Major injury have been seen following Humerus plating , ulna nailing , radius plating, external fixation of finger plating of phalanx  .Clinical photographs are presented to highlight the problem and their clinical management.
